State-by-State List of Nursing-Home Performance Data

Pennsylvania
The state's Long Term Care Gateway maintains a directory of nursing homes that provides basic contact information and important inspection results. Sort the directory by facility name, city, county, ZIP code, size, or ownership type.
Search results will reveal contact information, bed capacity, and the average number of nursing hours that a resident gets each day. Select the View Survey link to determine the date of a facility's last inspection, the results, and the nursing home's "plan of correction."
Click the View Additional Services link for more detailssuch as whether the nursing home will allow pet visits, if there is a barbershop or salon on the premises, and whether residents can participate in church services at the facility.

Rhode Island
The state's Survey Performance Tool for Rhode Island Nursing Homes identifies nursing homes alphabetically and indicates their survey scores in several categories such as Resident Rights and Food Service. (Note: The list does not provide specific contact information for nursing homes.)

South Carolina
South Carolina does not provide its own nursing home report cards online, but you can access this type of information by using Medicare's Nursing Home Compare tool to research Medicare- and Medicaid-certified facilities in the state.
The state provides a list of nursing homes (note: PDF file) that includes basic contact information at each facility.

South Dakota
South Dakota does not provide its own nursing home report cards online, but you can access this type of information by using Medicare's Nursing Home Compare tool to research Medicare- and Medicaid-certified facilities in the state.

Tennessee
Tennessee does not provide its own nursing home report cards online, but you can access this type of information by using Medicare's Nursing Home Compare tool to research Medicare- and Medicaid-certified facilities in the state.
The state provides a health care facilities directory, where you can search for nursing homes by county. The resource reveals contact information and bed capacity at each facility, as well as the date that the nursing home was last surveyed.

Texas
The state maintains a
Long Term Care Quality Reporting System that enables you to search for and evaluate nursing homes.
Choose the type of facility that you want to research, then identify the appropriate county, ZIP code, area code, or city name. The resulting list of nursing homes provides contact information, as well as an overall score and rating symbols that indicate a facility's general quality and performance during recent inspections. Click on the facility's name or any of the rating symbols for more details about its inspection results and quality of services.

Utah
The state offers online nursing-home report cards, but each facility's report shows up as a separate PDF file, so comparing different nursing homes will take some work.
Actually, each facility's report shows up as two PDF files1) a Full Facility Profile that covers the last inspection, and 2) a History Facility Profile that provides information from the last four annual inspections. The reports indicate the types of deficiencies that were discoveredand whether or not the problem was corrected. The state also maintains an alphabetical list of nursing homes in Utah that reveals contact information and bed capacity for each facility.

Vermont
The state's Department of Aging and Disabilities maintains a web site that is devoted to Nursing Home Information. Pull up the county-by-county list of nursing homes, a Guide to Choosing a Nursing Home, and a nursing home evaluation checklist.
The state also publishes a Nursing Home Resident Satisfaction Survey. The results for each facility reveal how it stacks up against the state average in categories such as dining, maintenance, nurses, activities, and more.
