Missouri
Missouri does not provide its own nursing home report cards online, but you can access this type of information by using Medicare's Nursing Home Compare tool to research Medicare- and Medicaid-certified facilities in the state.
The state's Department of Health & Senior Services provides an alphabetical Nursing Home Directory with names and addresses of nursing homes in the state. There's also a guide to choosing a nursing home in Missouri.

Montana
Montana does not provide its own nursing home report cards online, but you can access this type of information by using Medicare's Nursing Home Compare tool to research Medicare- and Medicaid-certified facilities in the state.
The state offers a facility search tool that you can use to search for nursing homes in Montana by city, county or ZIP code, or facility name.

Nebraska
Nebraska does not provide its own nursing home report cards online, but you can access this type of information by using Medicare's Nursing Home Compare tool to research Medicare- and Medicaid-certified facilities in the state.
The state offers a printable roster of nursing homes (note: PDF file), which contains facility information such as bed capacity, ownership, and names of key staff.

Nevada
The state provides survey results online via two PDF documents available for download: Consumer Information About Northern Nevada Nursing Homes and Consumer Information About Southern Nevada Nursing Homes. Each document reveals background information about the survey process, advice on choosing a nursing home, the date of each listed facility's last survey, and the survey ratings for each listed nursing home in several categories, including patient care and dietary services.
The state also offers two directories (note: PDF files) of nursing homes in the state: North Nevada Health Facilities and South Nevada Health Facilities. The two lists provide fundamental information such as bed capacity and contact information.

New Hampshire
New Hampshire does not provide its own nursing home report cards online, but you can access this type of information by using Medicare's Nursing Home Compare tool to research Medicare- and Medicaid-certified facilities in the state.
The New Hampshire Health Care Association offers a facility locator to identify nursing homes by city, county, or facility name. The search results reveal contact information and bed capacity.

New Jersey
The state maintains a very detailed Nursing Home Report Card database. Search for nursing homes by facility name, county, or city/townor by using the interactive state map. Your search results will provide basic information about each facility, as well as its "report card" scores in categories ranging from Administration to Environment. The state also offers a facility search tool to identify long-term care options by facility name, county or town/city, and facility type.

New Mexico
New Mexico does not provide its own nursing home report cards online, but you can access this type of information by using Medicare's Nursing Home Compare tool to research Medicare- and Medicaid-certified facilities in the state.
The New Mexico Health Care Association provides a directory of nursing homes that lets you search by facility name, city, or county.

New York
The state maintains a Nursing Home Information site that provides detailed information about nursing homes throughout the state. Click on the appropriate region of the interactive state map to find a list of counties in the region, then click on your county to generate a list of nursing homes in the area.
The list will reveal contact information and bed capacity at each facility. Each facility's notation also will provide links to:
- Facility Characteristics, which reveal the various services that the facility is certified to offer to its residents.
- Survey Summary Report, which identifies the date of the facility's last survey, its rating, and an assessment of any deficiencies. You can drill deep into details about specific deficiencies, and you even can obtain a copy of the actual survey that was completed for a particular facility.

North Carolina
North Carolina does not provide its own nursing home report cards online, but you can access this type of information by using Medicare's Nursing Home Compare tool to research Medicare- and Medicaid-certified facilities in the state.
The state provides lists of nursing homesa county-by-county list and an alphabetical directory, both of which show up as PDF files. The lists contain basic contact information for each nursing home.

North Dakota

Ohio
The state reveals inspection results within its Long Term Care Facility Search tool. Sort by facility name, city, county, or type of services needed to pull up a list of nursing homes to review. The results will reveal basic contact information, as well as the date of a nursing home's last inspection, the number of citations it received, and how it fared in comparison to other nursing homes in the state. Click on a facility's inspection date to view the nursing home's inspection report and the facility's response. (Learn about the various survey processes to better interpret the information.)

Oklahoma
