Florida
Florida's Agency for Health Care Administration provides a Nursing Home Guide that allows you to search for nursing homes in Florida by geographic region or by the characteristics of the facilities. Within the search results, click on Inspection Details for This Facility to see a list of a facility's citations from the past 45 months.

The agency also provides Nursing Home Watch Lists that reveal which facilities in Florida had deficiencies—and if and when these problems were corrected.

Georgia
Georgia does not provide its own nursing home report cards online, but you can access this type of information by using Medicare's Nursing Home Compare tool to research Medicare- and Medicaid-certified facilities in the state.

The state provides a Facility Location and Information Guide that searches by facility type, facility name, or location (county, city, or ZIP code).

Hawaii
Hawaii does not provide its own nursing home report cards online, but you can access this type of information by using Medicare's Nursing Home Compare tool to research Medicare- and Medicaid-certified facilities in the state.

The state offers a guide to long-term care options (note: PDF file).

In addition, the Hawaii Long Term Care Association's site provides information about long-term care, including a facility locator for the islands of Oahu and Kauai. The directory offers facilities' contact information, bed counts, and names of key staff.

Idaho
Idaho does not provide its own nursing home report cards online, but you can access this type of information by using Medicare's Nursing Home Compare tool to research Medicare- and Medicaid-certified facilities in the state.

Illinois
The Illinois Department of Public Health offers a comprehensive online guide to nursing homes in the state. The site explains how to select a nursing home and provides a directory of long-term care facilities.

The site also delivers quarterly reports of nursing home violations and a list of facilities that had no deficiencies in the most recent annual Medicare/Medicaid certification survey.

Indiana
The state's Department of Health's Long Term Care Division provides online report cards for all certified nursing facilities. Search by city or county for basic contact information, facility details, and performance data. (Get background information on the development of the report cards to help you interpret the results.)

Iowa
The state produces online report cards that let you search by facility name, city, or county to see contact information, staffing details, and performance data—from complaints or mandatory reports of abuse to recognition for the positive things that a facility has done for its residents.

Kansas
Kansas does not provide its own nursing home report cards online, but you can access this type of information by using Medicare's Nursing Home Compare tool to research Medicare- and Medicaid-certified facilities in the state.

The state offers a Residential Health Care Providers Directory (note: PDF file) that identifies basic information about nursing homes, such as addresses and bed counts.

Kentucky

Maryland
The Maryland Health Care Commission's Nursing Home Performance Evaluation Guide reveals facility-specific information such as bed counts, the characteristics of residents, and details about deficiencies that were found—if any—during recent state inspections. Search the directory by facility name, ZIP code, or county. The guide also provides maps to help you locate the facilities.

Massachusetts
The state's Survey Performance Tool for Nursing Homes lets you search for nursing homes by city or facility name to see scores and any deficiencies from the last three standard surveys. (Note: New nursing homes that have not yet experienced three surveys are not included.) You also can compare and evaluate nursing homes by their performance in five categories—Administration, Nursing, Resident Rights, Kitchen/Food Services, and Environment.

Michigan
Michigan does not provide its own nursing home report cards online, but you can access this type of information by using Medicare's Nursing Home Compare tool to research Medicare- and Medicaid-certified facilities in the state.

The state's Bureau of Health Systems offers a nursing home directory. Search the directory by facility name, city, county, or ZIP code—or by using the site's clickable map. The search results will provide contact information for each facility, as well as basic details such as bed capacity and types of care.

Minnesota
Minnesota does not provide its own nursing home report cards online, but you can access this type of information by using Medicare's Nursing Home Compare tool to research Medicare- and Medicaid-certified facilities in the state.

The state maintains a Health Care Facility and Provider Database, which you can use to locate nursing homes in Minnesota by county, city, or facility name. The search results will provide contact information, staffing details, bed capacity, and the types of resident care offered by each facility.

The Minnesota Board on Aging offers a nursing home evaluation checklist for you to use while reviewing facilities in the state.
